Please
try to anticipate your refill needs so that they can be taken care
of during your office visit. However, should this not be possible
please follow these guidelines.

First,
determine the number of refills remaining on the prescription. This
number is usually found in the lower left corner of the label. If
the number of refills left is 0, then the prescription is no longer
valid.
·
Second,
determine the date that the prescription was initially issued
(usually will be in the lower left corner of the label in
parentheses). IF your prescription is more than a year old, it
CANNOT be refilled and a prescription must be obtained. Under normal
circumstances, such prescription renewals should be obtained during
regularly scheduled visits with your doctor. In fact, one reason
that prescriptions have an expiration date is to remind you to make
an appointment with your doctor so the medication can be properly
monitored. Good medical care standards require that a doctor review
your medical record, and preferably examine you, before renewing an
expired prescription.

Third,
you should never allow your medication to completely run out,
specifically those medications you may be taking for a chronic or
long-term condition. Refills may require an office visit. Patients
with chronic illness should be seen for evaluation and monitoring at
a minimum of annually and often may require a visit every 6 months.

Fourth,
if your medication prescription amount (number of pills in the
prescription) has been limited,
that is at the request of the physician and most often requires an
office visit for additional prescriptions.
Before
calling your pharmacy or our office please remember:

New symptoms and/or events require an office
appointment. Our providers are unable to diagnose via phone.
·
No medication/prescription will be replaced
if lost, stolen, misplaced, or overused
Prescription
refill request should originate from your pharmacy. Please call your pharmacy to request your refill on
your medications. The pharmacy will then contact us with the refill
request listing the specific information that we require including
the medication name, dosage, frequency and last refill that was
performed. This reduces the time you may spend on the phone to
contact our staff that will in turn have to request the refill from
the pharmacy.

Refills
can take 24 to 72 hours to process.
o
Call
before the prescription expires.
o
No
nurse is available on the Refill Desk after 3 p.m.
weekdays or on weekends.
Prescription
refills may be denied
for medications in which there is a need for the patient to be
evaluated by the physician on a regular basis, including but not
limited to blood pressure, diabetic, pain or anti-depressive
medications. The pharmacists will notify you of the
denial and the need for you to call for an appointment in our
office.
Please
remember that it is ultimately your responsibility to have the
appropriate amount of medication on hand. If you come in for an
office visit you may request a 3-6 month refill on many of your
medications. This will reduce your anxiety and time in ordering
refills.
For
written prescriptions of Adderall, Ritalin and Concerta you will be
required to be evaluated by the physician no less than every other
month and there are no exceptions. For
these specific written prescriptions there
will be a $15.00 charge when you are not combining this
prescription with a physician visit.
There
will be a $15.00 charge for items for which the physician and/or
staff is requested to complete including but not limited to the
following items to be written or completed by the physician.
- 90
day written mail or fax in prescription forms
- Prior
authorization of medication through an insurance company.
